I'm guessing it's a Westfield / Columbia frame.
 
I agree, Columbia, and it looks to be in good shape. The serial number looks like it's not oriented correctly, but it indicates 1956, which would be about right for that frame. The head badge for that year has 2 vertical mounting holes, and looks like that shape on yours also.

Yes I was in the Jax area for awhile. I worked at Mayport for 9 years after I retired from the Air Force.
 
My "Goodyear" Highway Patrol (Columbia built) has the same stand mount, pretty common on available survivors. The earlier built in stands were not great*.

*(sucked);)
