Question that most needs answering: Is that an integrated hanger?

Assuming it is: What are my options for upgrading Tourney/Altus?

Lastly: I have heard bits on the Altus can be replaced with higher quality parts. Is this realisticly doable, and is it worth it?

I would call that an "axle mount" derailleur.
I have used them successfully on beach cruisers and muscle bikes.
That screw behind the axle must be kept tight or you can end up with a trashed derailleur and possible some broken spokes.
 
I'm pretty sure Altus requires the other type of hanger. Tourney TY300 has the rivet mount option and the other type. Be sure to get one like this

The hanger plate at your Amazon link would give you almost unlimited derailleur options. I have never used a hanger plate like that, myself.
