Internal geared fatty hub?

I was wondering if anybody made an internal geared (either 2 speed kickback or 3 speed) hub wide enough to use with a fatty rim.
A coaster brake would be preferred if possible...
 
Depends how fatty...not sure, but check the Sachs hubs or the sturmey archer.

Sturmey Archer made one for the 4" wheels on the Schwinn Spoiler and the three speed OCC.
Here is mine.

Yeah, like KF said, Sturmey-Archer makes'm; never seen one in coaster flavour, but they make a band brake version (SX-RB3) and a disc brake version (SX-RK3). These are 163mm and 170mm OLD, respectively.
